After opening the form, print it and then fill it out. Return the completed form to Julie Hewetson (Mitchell HS) or Brenda Montgomery, Karen McNichols, or Janie Anderson (BNL) by no later than 2 weeks after the Tea (the Tea date is here).

Your completed application must be no longer than the two pages provided. Do not attach pages to the application.
